Do you take insurance?
Yes!

Why should I schedule a consultation first?
Although it is certainly not mandatory, you are welcome to schedule a time to chat virtually, before deciding if we are a good fit for your needs. We are also happy to provide additional recommendations for other trusted providers during this consultation as well.
Depending on the services you are looking for, we can chat about scheduling an official visit and/or any other questions you may have.
-

_________________________
If you are seeking insurance reimbursement or plan to use insurance to cover the cost of services, a referral is typically required from your medical doctor for "SLP evaluation and treatment", as well as a corresponding medical diagnosis code, before your evaluation can take place.
The codes to discuss with your doctor or insurance provider (as applicable) will likely be the following codes, but please note these may change once we have our initial evaluation:
CPT Codes:
- 92524 Behavioral and qualitative analysis of voice and resonance
- 92507 Treatment of speech, language, voice, communication, and/or auditory processing disorders; individual
ICD10 Codes:
- ICD 10 therapy diagnosis codes (TBD by your medical provider)
Any additional documentation you can provide from previous SLP evaluations and/or relevant medical progress notes (Ear/Nose/Throat, Pulmonology, GI, etc) are very helpful in understanding specific medical history and prognosis / directions for voice therapy.
- You can ask your provider to submit both the referral and medical documentation in the following ways:
- Fax (preferred) (802) 448-5957
- Secure email: sierra@vtspeech.com
- Mail: 100 Main Street STE 4, Burlington VT 05401
Note: I am happy to collaborate with other medical providers and/or therapists as this is an important part of how I practice, but I will only reach out to other providers with consent (see below) and per HIPAA guidelines.
3. New Client Intake
You will find links to my intake forms below.
1. New Client Intake
2. Consent for Release & Use of Client Material (optional)
Once these forms have been completed and we have received a referral from your MD, you will receive an email with instructions on how to use our scheduling system to book an initial evaluation.
Please let me know if you have questions with any of this moving forward - I am always happy to be a resource.
I look forward to connecting with you soon.

This is a frequently asked question?
It all begins with an idea. Maybe you want to launch a business. Maybe you want to turn a hobby into something more. Or maybe you have a creative project to share with the world. Whatever it is, the way you tell your story online can make all the difference.
